   3. Voters ID

   5. Ration Card
2. Self attested copy as address proof of any one
   1. Latest telephone bill
   2. Latest electricity bill
   3. LIC receipt (if LIC policy is taken by applicant)
3. 2 Passport size photographs
4. Fill all forms with customer details
5. Demand Draft of Rs 8000/- in favour of “First Flight Tours & Travels” payable at "Mumbai" as non refundable processing charges (including cost of Digital Signature and our banner Size 6ft X 2ft)
